WHO IS ELIGIBLE?
To be eligible for an EPC or GPMP, a patient must have a chronic or terminal medical condition with or without complex care needs. Your condition must require at least 3 medical practitioners, including your GP.
A chronic health condition is defined as one that has been present for 6 months or more. It includes, but is not limited to, asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke.
Your GP will need to assess your eligibility for an EPC plan.
HOW DO I GET ONE?
To obtain an EPC or GPMP you need to make a standard appointment with your GP.
Please note that this referral takes 15 minutes, and cannot be done in a standard appointment when other issues are being discussed. If you are coming to see your GP about another issue, you will be required to book a long (30 minute) appointment or come back for another standard appointment.
To ensure continuity of care, it is important that you continue to visit your usual GP for your initial EPC plan and any reviews of your EPC plan.
